1. General provisions
- The regulations set out the conditions of participation in the workshop and safety rules for participants.
- The object of the workshop is to solve puzzles by wandering through the museum and expanding your knowledge of ancient pharmacy, medicines and treatments. At the end of the game, participants take part in a game of guessing the composition of the fragrance blends. Details will be provided in the offer of specific workshops.
- The organiser of the workshop is Przedsiębiorstwo Zaopatrzenia Farmaceutycznego CEFARM- Łódź Sp. z o.o. with its registered office in Łódź (94-406) at ul. KINGA GILLETTE 11 running the Museum of Pharmacy named after Jana Muszyński in Łódź at 2 Plac Wolności (“Organiser”).
- There is a fee to attend the workshop. The cost of participation and payment details will be given in the offer of the specific workshop.
- A minimum number of participants – 6 people – must register for the workshop to take place.
- Each group of participants attends the Workshop under the supervision of at least one adult group chaperone (“Group Chaperone”) provided by the person making the booking.
2. Workshop participants
- The workshop is designed for participants aged 13 and over.
- To participate in the workshop, a group of participants must register via the contact form on the website www.muzeumfarmacji.eu or in person at the Prof. Jan Muszyński Museum of Pharmacy. The following are the costs of the project and the payment of the fee. Places for individual workshops are limited. Group admission is determined on a first-come, first-served basis. Details on how to apply and payment will be given in the offer of the specific workshop.
- Cancellation of the workshop by the participant up to 10 working days before the scheduled date is possible only by sending an email containing the necessary details for a refund to the following address rezerwacjamf@doz.pl
- By registering, the person making the booking declares that he/she has read the attached information clause and agrees to the processing of his/her personal data as described therein, as well as obtaining the consent of the participants’ parents/legal guardians for their participation in the workshop.
3. Place and date of workshops
- The workshop takes place at the Museum of Pharmacy named after prof Jan Muszyński in Łódź, pl. Wolności 2 or at any other place indicated by the Organiser.
- The duration of the workshop may vary and will be stated in the offer of the specific workshop.
4. Rules for participation in workshops
- It is a condition of participation in the Workshop that the Group Supervisor provides written consents for the children to participate in the Workshop, signed by the parents/legal guardians of the participants, prior to the Workshop.
- The organiser provides all the necessary tools and products for the workshop, as well as expert supervision by the facilitator.
- The organiser provides in the description of the specific workshop a list of the ingredients of the products that will be used during the workshop.
- The Organiser may refuse participation in the Workshops to groups not meeting the age criterion of participants and groups without a Group Supervisor.
- The Group Supervisor is obliged to inform the Organiser of any circumstances that may affect the ability of their charges to participate in the Workshop. In particular, the Group Supervisor is responsible for checking that his/her charges are not allergic to any of the ingredients used in the Workshops. In the event that the Group Supervisor fails to inform the Organiser well in advance of the occurrence of an allergy to any ingredient in the product, the Organiser shall not be responsible for any complications in the event of any symptoms of allergy or intolerance. The organiser does not provide insurance for Participants.
- Workshop participants are required to behave in a manner that does not endanger other workshop participants.
- It is forbidden to bring or possess dangerous objects into the Workshop.
- Participants in the workshop are obliged to comply with health and safety regulations, rules and regulations and strictly follow the instructions and recommendations of the facilitators and the Organiser. In particular, workshop participants may be required to wear protective clothing, gloves, clip long hair, etc. The Group Supervisor is responsible for the behaviour of his/her charges and for any damage caused by them.
- Workshop participants are obliged to keep the work area and the equipment and devices they use during the workshop clean and to use them in accordance with the instructions for use. In the event of damage or destruction of equipment, tools, devices, furnishings of the workshop venue, if these are the result of negligence or careless/reckless or reckless behaviour of the participant, the participant is obliged to cover the costs of repair or purchase of new equipment.
- The organiser shall not be held liable for any damage to health or injury resulting from failure to observe health and safety regulations, the rules and regulations or to comply with the instructions and recommendations of the instructors and the organiser.
- Misbehaviour by a participant that disrupts the safety of the event may result in the termination of the participant’s participation in the workshop without refund of the fee.
- In the event of an emergency during the workshop, participants are obliged to notify the facilitators immediately.
5. Complaints
- Any complaints relating to the workshop can be made to the e-mail address: muzeumfarmacji@doz.pl within 14 days after the workshop in question has taken place..
- In the complaint, he or she should state his or her name, mailing address, as well as the nature and date of the workshop-related irregularity.
- The organiser undertakes to consider the complaint within 14 days of its receipt and to inform the complainant of its resolution to the complainant’s e-mail address.
6. Final Provisions
- The organiser reserves the right to interrupt, change or extend the workshop for reasons beyond its control.
- By participating in the workshop, the participant accepts the content of these Rules.
- Any disputes that may arise from participation in the workshop will be settled by the court having jurisdiction over the Organiser’s registered office.
- In matters not covered by these regulations, the provisions of generally applicable law shall apply.
